Vancomycin should be stopped as soon as Gram stain/culture indicates N. meningitidis, or H. influenzae, or if ceftriaxone resistance excluded.

Desired vancomycin trough is 10-20mg/L. Monitor renal function closely.  See Vancomycin Dosing & Monitoring Guidelines.
