Use imipenem or meropenem if:

• suspected infection with ESBL or Amp C producing organism
• nonsevere penicillin allergy

Add vancomycin (alternative linezolid) if:

• suspected catheter-related infection, SSTI, pneumonia
• AML patients with mucositis and persistent (> 48h) fever
• previous/current colonization or infection with MRSA

Add linezolid instead of vancomycin if:

• previous/current colonization or infection with VRE
