• Note:  previously recommended target of 15-20mg/L has been analyzed in numerous studies and not found to be consistently associated with improved outcomes, but does correlate with nephrotoxicity risk. Therefore, a specific target trough range for severe infections is undefined and attainment of such should be subject to careful consideration of patient risks and benefits.  There is little to no evidence to suggest a more specific target range than 10-20mg/L.
