Addition of gentamicin and rifampin has been challenged due to insufficient evidence of benefit and known increased risk of adverse events. Use of these adjunctive agents requires consultation with Infectious Diseases specialist.

To prevent the development of rifampin resistance, rifampin should be started 3-5 days after vancomycin and gentamicin.
