Administration of Amphotericin B deoxycholate
Infusion
Peripheral line- infuse dose over 4-6 hours
- max. concentration 0.1 mg/mL
Central line
- infuse dose over 2-4 hours
- max concentration 0.25mg/mL
Premedication
Routine premedication NOT recommended
May be given if infusion-related reaction occurs & then as pretreatment with subsequent doses.
Fever/chills/rigors
Meperidine 25-50 mg IV
Fever
Acetaminophen 650mg PO (max 4 g/day)
Nausea/vomiting
Diphenhydramine 25-50 mg PO/IV
Give premedication 30 min. prior to amphotericin B deoxycholate
AVOID OTHER NEPHROTOXINS (e.g. aminoglycosides, calcineurin inhibitors [cyclosporine, tacrolimus], cisplatin, foscarnet, pentamidine, tenofovir)
