Gentamicin	7mg/kg IV q24h

If high institutional MRSA rate

Vancomycin	60mg/kg/d IV div q6h	≥ 7 days
+
Gentamicin	7mg/kg IV q24h

Severely ill

Empiric Therapy	Dose	Duration
Piperacillin-tazobactam	240-300mg piperacillin/kg/d IV div q6h	≥ 7 days
If meningitis not ruled out:
Meropenem	120mg/kg/d IV div q8h	≥ 7 days

If high institutional MRSA rate, add
