If daptomycin MIC ≤ 2µg/mL.

For daptomycin MIC ≥ 4 µg/mL, clinical failure may occur even with high-dose.  Infectious Diseases consultation recommended. 

If daptomycin MIC ≥ 8 µg/mL, use linezolid.
