Hepatosplenic candidiasis/Chronic disseminated candidiasis

- Patients receiving ongoing chemotherapy or stem cell transplantation should continue antifungal therapy throughout high risk period to prevent relapse.

 
Usual Pathogens

Candida spp

 

Empiric Therapy Dose Duration
Amphotericin B, lipid complex or liposomal
3-5mg/kg IV daily

3-6 months and resolution or calcification of radiologic lesions. 

Can switch to fluconazole 400mg (6mg/kg) PO daily if patient clinically stable and isolate susceptible. 

or    
Anidulafungin

200mg IV once then

100mg IV daily

3-6 months and resolution or calcification of radiologic lesions. 

Can switch to fluconazole 400mg (6mg/kg) PO daily if patient clinically stable and isolate susceptible. 

or    
Caspofungin

70mg IV once then

50mg IV daily

3-6 months and resolution or calcification of radiologic lesions. 

Can switch to fluconazole 400mg (6mg/kg) PO daily if patient clinically stable and isolate susceptible. 

or    
Micafungin 100mg IV daily

3-6 months and resolution or calcification of radiologic lesions. 

Can switch to fluconazole 400mg (6mg/kg) PO daily if patient clinically stable and isolate susceptible.
