Rule out S. aureus bacteremia. Linezolid preferred over vancomycin if:

• MRSA with vancomycin MIC ≥ 1.5µg/mL
• concomitant nephrotoxic therapy
• pre-existing renal dysfunction

If culture proven MSSA, switch to cloxacillin or cefazolin.
