  Refer to local guidelines for dosing & monitoring.

• Infants > 44 weeks PCA & children

  2.5mg/kg (IBW) q8h

• Cystic Fibrosis

  2.5-3.3 mg/kg (IBW) q6-8h

• Synergy

  1mg/kg (IBW) q8h
