Rule out S. aureus bacteremia/endovascular infection. Consider MRSA if:

• rapid onset
• post-influenza
• IVDU
• colonization/recent infection with MRSA
• Gram positive cocci in clusters on Gram stain

Linezolid preferred over vancomycin if:

• MRSA with vancomycin MIC ≥ 1.5µg/mL
• concomitant nephrotoxic therapy
• pre-existing renal dysfunction.
