Dosing Interval Determination for Gentamicin/Tobramycin/Amikacin
Pediatric patients
Give q24h. If serum level is necessary (see Aminoglycoside serum concentrations), adjust interval of subsequent doses, if need be, as follows:
- if interval level drawn - adjust interval according to the Hartford Nomogram, or
- if trough level drawn and > 0.5-1mg/L - lengthen dosing interval or consider alternate antibiotic.
Adult patients
Calculate creatinine clearance (Clcr); determine dosing interval according to chart below.
Calculated creatinine clearance (mL/min)
Clcr (females) = [(140 - age) x IBW] divided by Scr (µmol/L)
Clcr (males) = Clcr (female) x 1.2
