Add vancomycin if MRSA colonized, or operative cultures show Gram positive cocci in clusters/clumps on Gram stain and patient not improving.

Desired vancomycin trough is 10-20mg/L. Monitor renal function closely. See Vancomycin Dosing & Monitoring Guidelines.
