Consider HSV2 if ≥ 2 episodes of culture negative meningitis with lymphocytic predominance of CSF - send CSF for Herpes Simplex PCR.  Treatment of acute episodes with acyclovir is controversial, unless significant neurologic symptoms (urologic retention, weakness).  Prophylaxis with antivirals has not been found to prevent recurrences.
