If C&S results unavailable, enterococcal coverage with ampicillin (in addition to Gram negative coverage) is recommended for high-risk patients:

• elderly/institutionalized

• catheterized

• obstruction and/or anatomical abnormality of GU tract

• diabetes

• prosthetic heart valve.
