Adult > 50 years or Immunocompromised (including AIDS), Alcohol abuse, Debilitating illness, Pregnancy

- HIV individuals - need to rule out:
- Cryptococcosis
- M. tuberculosis
- Syphilis
- Toxoplasmosis
