Use linezolid if colonized/infected with vancomycin-resistant Enterococcus spp.  Oral linezolid has excellent absorption and bioavailability. With equal doses, the serum concentration attained with oral linezolid is the same as that of the IV formulation. Therefore, unless the patient is strictly NPO, use oral formulation.
