Oral metronidazole has excellent absorption. If intravenous therapy is deemed necessary, prompt switch to the oral formulation is recommended. (See IV to PO Switch Recommendations).

Anaerobic coverage with metronidazole recommended if:

• severe ischemia
• foul-smelling discharge
• necrosis
• gangrene
